RevUp is a Medicare-covered care management benefit that connects you with your own dedicated Health Coach, giving you the tools, support, and guidance to take control of your health every day—not just when you’re at the doctor’s office.

When you enroll in RevUp, you’ll be paired with a RevUp Health Coach who will work with your provider to offer:
Personalized care plans tailored to your unique health needs
Ongoing support to help you manage conditions like diabetes, high blood pressure, and heart disease
Guided tracking of health data, including blood pressure, glucose levels, food intake, and physical activity
Regular progress reviews by your health coach and provider
